I have a 7.1.2 site containing a document module hosting PDF's the document name is 2016-02-04 when I open the document it opens in a new browser window but the name in the tab is incorrect I've tried several things but no luck changing it also where can I set the icon to display on any new tabs when documents are opened.

Very strange that date title! I have no idea of where is that coming from, however I have 2 suggestions:

1 - Check the content of the table "Documents" and see if you can find anything strange there in regards to that record. You can run the following command under Host/SQL:

select * from Documents 

2 - Then also check the meta information where that PDF was created from. Maybe there is something there - http://www.prepressure.com/pdf/basics/metadata
